We will be happy to register anyone who lives within the practice boundary shaded in blue as per the map under the practice Boundary tab.
In order to register you will need to complete a New Patient Registration Form (GMS1), practice Health Questionnaire and Summary care Records form, which are available below.
Please bring the completed forms, 1 proof of address and 1 proof of ID with you when you come to register. If you were registered with a GP in the UK before please provide us with your NHS Number (National Health Services not NI Number ) to help us to register you correctly. Children under 6 years old being registered must provide us with the baby red book or a record of their immunisation if they have entered from outside the UK. Children under 6 years old registering must book an appointment with the Practice Nurse.

- reception staff will check if all documents are completed correctly,- check your proof of address and ID and will ask you to use the surgery pod to check your BP, Height and Weight.
- FREE NHS Health Checks If you are age between 35 and 74 with No pre-existing medical conditions (e.g. heart conditions, diabetes, Cholesterol, Blood pressure medication) then we would like to invite you for a FREE NHS health check.

In order to book the health check please phone the surgery to make an appointment with Maxine, our practice nurse (2-3 weeks advanced), as you will need to have blood test first before the initial appointment.
