Please note that you need to collect a online appointment registration letter from the surgery in-person (due to confidentiality we require patients to collect in-person). Please bring photographic ID with you.

Telephone requests will only be accepted from housebound patients.

If you have mislaid your repeat slip there are forms available at reception.

Medication Reviews

Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at least once a year to review these regular medications and notification should appear on your repeat slip.

Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
